WINSTON-SALEM, N.C. – In partnership with health systems, health departments and nonprofits across central North Carolina, Novant Health is currently conducting a 2025 Community Health Needs Assessment (CHNA) to help identify and understand community health challenges and collaborate on meaningful solutions. The information collected by this survey and the CHNA process will help guide programs and resources to meet the needs of community members.
The survey takes less than 10 minutes to complete and may be accessed online here. Those 18 years old or older are eligible to participate. The deadline for county residents to participate is April 30.
Comprehensive community health needs assessments help Novant Health engage communities in the strategic community benefit planning process to help ensure patients across communities we serve receive intentional and remarkable care. About the CHNA: The Community Health Needs Assessment is a vital process conducted every three years to help leaders identify priorities to drive community investment, programming and policy efforts. A key component of the process is a community survey to capture the voices and perspectives of local residents. In addition to the survey, the assessment process also includes a review of secondary data, key informant interviews and focus group discussions. Local coalitions will review all of this information to identify priority areas and develop a roadmap for local action.
About Central Carolina Community Collaborative: The Collaborative’s purpose is to amplify our local community voice through the Community Health Needs Assessment (CHNA) process. The Central Carolina Community Collaborative brings together six health systems (27 hospitals), 11 health departments and four United Way agencies. The collaborative work is centered around: aligned CHNA cycle timing, consistent data collection (both primary and secondary standardized elements with county-level customization available), report writing and formatting (providing a standardized template that each county can then customize), and resources to support funding and sustainability.
