CHARLOTTE, N.C. – Novant Health recently launched Epic’s MyChart Care Companion, an interactive, highly individualized plan of care delivered to at-risk patients through user-friendly, accessible technology. Care Companion offers a roadmap for patients managing a chronic condition or navigating their follow-up care after a procedure.
“More than 1 million Novant Health patients are using MyChart to communicate with their care team,” said Dr. Keith Griffin, chief medical information officer for Novant Health Medical Group. “After offering this patient portal for many years, we are excited to extend the support our patients have come to expect by adding tools like medication adherence and symptom tracking for those who are recovering from a procedure or managing a chronic condition.”
Novant Health currently offers Care Companion for pregnancy, type 2 diabetes, heart failure, hip and knee replacement and asthma.
Care Companion focuses on the delivery of timely education and includes system check-in questionnaires to check for potential areas of concern. Patients receive push notifications on their mobile device when it’s time to complete a task. If a patient misses a task, or if a response to the task indicates a potential problem, Care Companion notifies the clinical team to follow up with a patient, adjust the care plan if needed or forward to the provider when medical advice is needed.
For example, with Care Companion Healthy Pregnancy, expectant mothers with low-risk pregnancies receive timely education throughout the course of their pregnancy, including information about baby’s growth, healthy eating, exercise and preparing for labor and delivery. Expectant mothers also complete symptom check-in questionnaires to check for preterm labor or other areas of concern.
