Novant Health launches program with prescriptive analytics leader Jvion

WINSTON-SALEM, N.C. – Last year, as part of the Novant Health Institute of Innovation & Artificial Intelligence, Novant Health announced a partnership with Jvion, a leader in artificial intelligence (AI)-enabled prescriptive analytics for preventable harm. This month, the partnership allowed Novant Health to launch the congestive heart failure readmission reduction program, which was implemented at Novant Health Forsyth Medical Center and Novant Health Presbyterian Medical Center. 

This program’s goal is to reduce readmissions for congestive heart failure (CHF) patients by 5%. The readmission rates for CHF patients can be as high as 35% in the first year and can double with each subsequent hospitalization. By integrating clinical and socioeconomic data and leveraging Jvion’s AI technology, Novant Health aims to identify the most relevant interventions for the patients most likely to respond to them to prevent a readmission.

The congestive heart failure readmission reduction program will eventually be expanded to all of Novant Health’s hospitals.

"Heart failure is one of the leading causes of hospitalization for older adults," said Dr. Gordon Reeves, Novant Health director for advanced heart failure. "Patients hospitalized for heart failure are inherently a high-risk population and are more likely to have a diminished quality of life after hospitalization. Jvion may allow us to focus our resources on helping them have a successful transition out of the hospital and recover to a better quality of life."

Novant Health will leverage Jvion’s AI Machine learning algorithms to help prevent patient harm and reduce associated costs by enabling clinical staff to focus attention, resources and individualized interventions on patients whose outcomes can be improved. Unlike traditional AI and predictive analytic solutions that merely identify high-risk patients and cause alarm fatigue, Jvion’s model pinpoints the specific patients who are on a risk trajectory that can be changed and provides the individual patient recommendations that will drive better outcomes. Based on data from 16 million patients and a unique use of Eigen-based models, Novant Health will be able to quickly apply this technology to CHF patients. The solution is deployed on the Microsoft Azure cloud and will further strengthen Novant Health’s existing partnership with Microsoft.

“This partnership with Jvion supports our focus of identifying advanced technologies that provide highly personalized care with actionable data and insights for preventive prediction, diagnosis and treatment,” said Bipin Karunakaran, vice president of clinical insights and analytics, Novant Health. “By using more than 4,500 clinical and socioeconomic factors, Jvion’s technology can identify which patients will need intervention and provide high priority clinical and socioeconomic intervention steps to prevent readmission.”

Jvion has successfully reduced CHF readmissions by 13% on average, preventing 130 readmissions per 1,000 discharges for a potential savings of approximately $1 million. Jvion’s AI Machine learning algorithms identifies individual patients at risk early and prevents their risks from rising, thereby reducing avoidable harmful events; the clinical and socioeconomic determinants create personalized interventions that change a patient’s risk trajectory.
